New Passport/Child Passport
Follow these instructions if:
You are applying for a new passport/child passport for the first time.
You are applying for a minor (under 18 years old).
Your passport was issued more than 15 years ago.
Your passport was issued before you turned 16.

In order for our company to process your passport application, EVERY applicant must print out numbers 1 and 2 below.
Requirements
1) Release letter ( must print out )
2) Passport & Visa Specialists Order form
To use our service you MUST fill out the ONLINE Department of State passport application below, click the Create button at the bottom and THEN print the form.
If you do not use the form below your passport application will be delayed.
3) DS-11 passport application
4) Two identical passport photographs taken by a passport photographer (Kinko's and Walgreens take these).
5) Original Certified Birth Certificate or naturalization certificate or the most recent passport. If applying for a Child passport and you are providing a previous passport for your child you must include a certified birth certificate in addition to the previous passport to prove who the child's parents are.
6) 1 Check/money order made payable to Department of State for $135.00 for adults or $120.00 for minors and another check for $25.00 for the post office/county clerks fee. No temporary checks will be accepted by the passport office. You can use a money order in place of a temporary check.
The following applies to customers who ordered a birth certificate and passport from our company.
***If you ordered a birth certificate and passport from our company the governments fee for a passport is not included in our passport/birth certificate service fee of $175. The $175 fee you previously paid covers our service fee for a birth certificate ($59), service fee for a passport ($91) and return delivery by Federal Express ($25).***
7) Valid Picture Drivers License issued more than 6 months ago (no paper licenses). If your Driver's License has been issued or renewed in the last 6 months, you will need to provide at least 3 personal documents. All applicants must have either a valid drivers license or a valid paper ( temporary) drivers license. If applying for a child, the parents license is what is needed.
8) If you need a passport within 10 business days then you will also need proof of departure which will be ONE of the following:
a) copy of your airline tickets or
b) travel itinerary or
c) company letter of expedite (sample)
9 ) Both parents must appear with the child at the acceptance agent when obtaining the sealed envelope. If both parents cannot be present then a Written Statement of Consent must be provided to the acceptance agent. (The Written Statement of Consent must be notarized.)
In order for us to process your new passport / child passport you must go to a local acceptance location ( Every person must appear in person as of 6-17-04 ) which will either be a special post office or county clerk's office.
Make sure you take sufficient identification such as a driver's license with you. You will need to show the items listed above which will then be put into a government sealed envelope. Make sure you get the envelope back from the clerk and do not open it. Once you have your sealed envelope you will need to Federal Express the sealed envelope and the Passport & Visa Specialists Order Form with the payment for our services to the address on the top of the Order form.
After you appear at your local acceptance location you will then Federal Express your sealed envelope, order form and payment to us at:

1. Sealed Envelope
2. Our Order form filled out completely
3. Payment to Passport and Visa Specialists*
