Requirements
If your passport is over 1 year old you must follow the passport renewal instructions. Click Renewal on the left of this page.

Follow the instructions below if you were issued a passport that had a validty that is shorter than the full validity. Most of these will be if you lost your passport while overseas or did not have a certified birth certificate when you applied for your passport.

1) Completed DS5504 application form.

2) Release letter (must print out)

3) Please fill out the order form and print a copy to send to us inside your Federal Express Priority Overnight.

4) A letter that must state the request for the extension of the limited passport and the reason why ( conflicting countries or frequent travel ).

5) Government Fee - $60.00 No temporary checks will be accepted by the passport office. You can use a money order in place of a temporary check.You can write one check for the governments fee, our service fee and return delivery by Federal Express. ( For the correct amount please see the order form.)

6) If you are leaving the country within 10 business days, then you will also need proof of departure, which will be ONE of the following:

a) copy of airline tickets or

b) travel itinerary or

c) company letter of expedite

7). Original Certified Copy of your Birth Certificate or a valid passport.

8). Copy of your Valid Drivers License

9) You do not need to make a personal appearance at the acceptance agent.
